Team-based care involves health professionals from different disciplines working collaboratively on a care team with the patient and the patient’s primary care provider to enhance the patient’s care. Team-based care, as part of a comprehensive treatment protocol for hypertension management, is a best practice strategy that can help improve blood pressure control, particularly among patients who are African American, Black, Hispanic, or Latino. In this webinar, we provide strategies and best practices for using team-based care to improve blood pressure control rates. Presenters will also provide an example of team-based care as part of a larger strategy to improve patient hypertension via telehealth.

Cancellation Policy: Effective 11/1/21 - No refunds will be available for cancellations that are received within 10 business days before the start date of the event/training. To receive a maximum refund of 50% of the registration price prior to this deadline, all cancellations must be in writing and submitted by emailing training@cpca.org. Substitutions of registrations in lieu of cancellation is encouraged.
