Team-Based Care to Improve Blood Pressure Control

Team-based care involves health professionals from different disciplines working collaboratively on a care team with the patient and the patient’s primary care provider to enhance the patient’s care. Team-based care, as part of a comprehensive treatment protocol for hypertension management, is a best practice strategy that can help improve blood pressure control, particularly among patients who are African American, Black, Hispanic, or Latino. Join us as we hear from a community health center that employed team-based care as part of a larger strategy to improve patient hypertension via telehealth.

Cancellation Policy: Effective 11/1/21 - No refunds will be available for cancellations that are received within 10 business days before the start date of the event/training. To receive a maximum refund of 50% of the registration price prior to this deadline, all cancellations must be in writing and submitted by emailing training@cpca.org. Substitutions of registrations in lieu of cancellation is encouraged.
